Medical Release Information
Each teen must have a filled out and signed medical release form to attend Fire & Ice. If you are dropping your teen off in person, we will collect your information from your registration form and have your medical release form ready for you to sign. If you are not dropping off your teen, you must print, fill out, and sign this medical release form.
